It's a Process
Your health is multifaceted and is a continual process. While some patients may respond faster than others, we will work with you to help achieve your goals and educate you to achieve long term health improvements. We will work with you to help you achieve your optimal health.
CONSULTATION FEES:
NEW PATIENT APPOINTMENTS: 60 minutes $200
FOLLOW UP APPOINTMENTS: 30 minutes $100
CONSULTATION PROCESS:
- Please call the pharmacy to schedule an appointment 856-829-4877
- You will be asked to complete a Medical History & Health Appraisal Form prior to your appointment.
You can select the appropriate electronic form below. - We will consult and communicate with your physician.
- We will make recommendations to your physician if necessary.
- If a treatment plan is started, we recommend a follow up consultation between 1 to 3 months to evaluate the current therapy and make any suggestions.
- We are always available to answer questions.
